can anyone remember...

a book that takes place over the course of one breakfast at a house of pancakes. A group of pyschologists, the protagonist has an outer body experience, remembering an affair with a waitress, maybe? I remember reading it in one day, and it was really absorbing, but that's all. It might have been green, and pocket sized and written by a man.
